New York, NY— The health care delivery system in the United States has long been described as a “cottage industry”. There is a fragmentation at the national, state, community, and practice levels. This fragmentation contributes to the poor overall performance of the U.S. health care system. Reforming health care payment and delivery and expanding coverage are not only complementary, but each is critical to achieving the other.
With that, SOMOS hosted their first annual conference titled, Neighborhood Based Primary Care: An Innovative Organizing Principle for Healthcare Delivery. They partnered with New York State Budget Director Robert Mujica, New York State Office of Alcoholism and Substance Abuse Services Commissioner Arlene Gonzalez-Sanchez and New York City Department of Health, Mental Hygiene Commissioner Dr. Ann Marie T. Sullivan, and former New York Medicaid Director Jason Helgerson, along with other key community leaders. In addition, MDLand joined local public health, community health, and delivery system partners and providers in the various hosted sessions, panels, and activities. They discuss models for a more effective integration of social and health systems to address social determinants of health.
MDLand International delivers one of the major EHR platforms in NYC. They are a leading health information technology company. This mission is to provide cloud-based certified electronic health records, practice management systems, clinical connectivity and integration, and population health platform to practices and healthcare organizations. MDLand joined hundreds of diverse doctors who serve over 700,000 patients through Governor Cuomo’s DSRIP initiative in attendance. They mapped out a long-term strategy to address severe health disparities and cultural barriers to care. Their efforts are to improve New York’s healthcare delivery system, to increase care and drive down costs. To accomplish this, they will improve connections to care and healthcare record sharing.

Uniting Communities Through Culturally Competent Care and Policy Reform
SOMOS’s First Annual Conference outlined operational best practices and mapped a long-term policy agenda. This agenda addressed health care disparities in New York’s Immigrant Neighborhoods, with a concentration on Latinx, Chinese Communities, to enhance the connection between patients and providers. Dr. Ramon Tallaj, SOMOS Founder and Chairman, and a practicing physician with practices in Upper Manhattan spoke as well. He is an immigrant himself from the Dominican Republic, said “as physicians on the frontlines serving over 700,000 Medicaid beneficiaries, including over 500,000 in the Latinx community, we have seen firsthand the impact of the DSRIP initiative. It helps make significant strides to improving New York’s healthcare delivery system. It also increases care and driving down costs through improved connections to care and healthcare record sharing. However, the work is far from finished”.
Dr. Henry Chen is President of SOMOS. He is also a physician with practices in Queens, Brooklyn and Manhattan, and a Chinese immigrant himself. He noted, “We want to build on our progress to strengthen our communities which, too often, are isolated from advancements in healthcare.

Together, MDLand works with local organizations to improve connectivity between physicians with the goal of improving patient outcomes.
