Technology also plays a crucial role in TCM. Using predictive analytics, healthcare organizations can identify patients most at risk during transitions and proactively support their care needs.

Comprehensive TCM must also address SDOH, which impacts patients’ ability to engage in follow-up care. SDOH analytics tools aid providers in identifying potential barriers, such as lack of transportation or inadequate social support, and integrating these insights into personalized care plans. This holistic approach promotes equity in patient care, ensuring that at-risk populations receive adequate support throughout their healthcare journey.
