The Centers for Medicare and Medicaid Services (CMS) Primary Care First (PCF) program is a new voluntary alternative five-year payment model that rewards value and quality of care by offering an innovative payment structure to support the delivery of advanced primary care. Primary Care First aims to enhance quality of care, improve patient experience of care, and reduce expenditure.
MDLand is committed to supporting the value-based program via our VBP-ready iClinic® EHR, as well as population health management platform. Of the 6 CQM measures needed to qualify, MDLand is now eligible for all measure requirements.
MDLand customers can participate in the PCF program.
PCF benefits/incentives:
- Opportunity to increase revenue with performance-based payments that reward participants for reducing acute hospital utilization
- Access to actionable, timely data to inform care transformation and assess your performance relative to peers
- Focus on a single outcome measure that has significant impact for patients
- Enhanced payments for practices that specialize in complex, chronic patients and high need, seriously ill populations
- Potential to become a Qualifying APM Participant by participating in an Advanced Alternative Payment Model
- Less administrative burden and more flexibility so providers can spend more time with patients and deliver care based on patient needs
PCF participating providers may receive additional revenue based on their performance and is only available in certain states. Currently, Primary Care First is offered in 26 regions: Alaska, Arkansas, California, Colorado, Delaware, Florida, Greater Buffalo region (New York), Greater Kansas City region (Kansas and Missouri), Greater Philadelphia region (Pennsylvania), Hawaii, Louisiana, Maine, Massachusetts, Michigan, Montana, Nebraska, New Hampshire, New Jersey, North Dakota, North Hudson-Capital region (New York), Ohio and Northern Kentucky region (statewide in Ohio and partial state in Kentucky), Oklahoma , Oregon, Rhode Island, Tennessee, and Virginia.
